England spin bowler Giles went to see a consultant on Tuesday and is doubtful for the start of the county season.
The 33-year-old did not play at all last summer because of hip and groin injuries and underwent two operations.
He returned to action during the winter Ashes series, but experienced further discomfort on Warwickshire's pre-season trip to the Caribbean.
Giles was confirmed as deputy to Warwickshire skipper Heath Streak last month and said he was looking forward to taking on extra responsibility.
"We have a strong squad in place and I know we can be challenging for trophies this year," he commented.
News of his latest injury problem is a blow to both Warwickshire and to Giles's chances of earning a recall to the England side after losing his place to Monty Panesar mid-way through the series in Australia, which they lost 5-0.
He later had to return home from the tour when his wife was diagnosed as suffering from a brain tumour.
